Washing and disinfection steps: (1) 1st washing 2’30” (2) enzyme washing 5’ (3) 2nd washing (4) disinfection 10’ (5) the last washing 2’

Cleanser: 1:400 enzyme lotions (produced by Nikon Company, batch number20051111)Disinfector: 2%intensified glutaraldehyde bactericidal agent (produce by Nikon Company, batch number 20060109)

[Checking equipment] sampling lotion: 0.03%PBS+0.1%sodium sulfite (neutralizer)

Culture medium: common nutrient agar

 Else: 90mm plate

[Checking date] 3/13/2006(use 2% glutaraldehyde at the fourth day), 3/14/2006(renew 2% glutaraldehyde)

[Checking measure]1. Sampling from endoscope internal lumen: inject 5ml sample liquid into endoscope lumen use impregnator; stirring endoscope and let the sample liquid contact the inner wall of lumen well; pouring sample liquid into agar plate and put away redundant liquid.

2. Sampling from endoscope surface: choose three tube sections, take print on the agar surface, each sample area is 3cm×6cm.

3. Record endoscope number and washing worker number.

[Checking conclusion] this check data show that wash and immerse endoscope in 2% glutaraldehyde in steps can achieve the standard.

[Correlative suggestion] 1. It is necessary to change clean glove when fetch endoscope from glutaraldehyde bactericidal agent.

2. Renew timing if add endoscope after endoscope immersion.

3. According to the workload of enteron endoscope diagnose, we suggest customers to replace 2% glutaraldehyde in five days.

4. The endoscopes washing and disinfection in different department must be check respectively.

“The endoscope washing Effect observation of Kangaking endoscope washing system and CLO2 disinfector ”
 Result Report

This observation is based on the flow request of disinfection technology norm and operation norm of endoscope washing and disinfection (2004) passed by minister of health. Washing in four troughs and disinfecting once, use high pressure water gun, air gun, pipe irrigation machine to reinforce washing. Choose 150mg/l CLO2 disinfector (produced by Changzhou Shimin chemical industry Ltd. Company); immerse 5 minutes to disinfect, use deionized water to wash. Sampling based on operation norm of endoscope washing and disinfection (2004), 34 times and 68 samples. The bacterial culture result showing: The endoscope washing Effect of Kangaking endoscope washing system and CLO2 disinfector is affirmative (look at table 1). At the beginning, because of the misapplication of pipe irrigation washing, otology influenced the disinfection effect and the cultivate outcome didn’t reach the standard. But after uniform training and communication among doctors from gastroscope department, respiratory department and otology, infection control office give advanced advices according to norm and technologists’ discussion. The washing and disinfection process is normalization and the qualification rate is promoting, the qualification rate reached 100% at later stage (no pathogen growing). Accessories are washed by ultrasonic washing machine which produced by Jiangsu Kunshan ultrasonic instrument Ltd. Company, pressure steam sterilizing reach the standard.

Suggestions: softy endoscope washing and disinfection process confirmed that use environmental and cheaper CLO2 replace the expensive and poisonous glutaraldehyde. To assure the safety of patients, workers and environmental, accessories should washed by ultrasonic washing machine and disposed by pressure steam sterilizing.

 Microbes monitor report of Endoscope environment
 

一、monitor time: April,2005~August,2005

二、Endoscope washing, disinfection flow: performed according to <operation norm of endoscope washing and disinfection>passed by minister of health in 2004, and the washing process adopt “kangaking” washing center.

三、Monitor measures:

1endoscope body

Sampling: sampling after sterilization and before use. Embrocate the endoscope body (30cm below endoscope month) use cotton cloth which contains sterile eluent, and then turning the sample cotton cloth, cut away the part which operator has touched. Put the sample into sterile eluent test tube which contain relevant neutralization agent, and submit for censorship instantly.

Monitor measure: shacking the sample liquid at least 80 times, add 1ml sample liquid to sterile dish (diameter is 90mm.), and then add nutrient agar, shacking while injecting, put it into 35 centigrade incubator for 48 hours and count after the agar clotted.

()the inner side of endoscope lumen

Sampling: monitor the inner side of endoscope lumen. Draw out 10ml buffer solution which contain corresponding neutralization agent use sterile injector, and then inject it into endoscope from active mouth, collect it use 15ml sterile test tube from active outlet. Submit for censorship in tow hours.

Colony count: shacking the sample liquid at least 80 times, add 0.5ml sample liquid into two sterile dish (diameter is 90mm.), each of plate add 15ml~18ml melted nutrient (45C~48C), shacking while injecting, put it into 35 C incubator for 48 hours and count after the agar clotted.

Result judgment : colony quantity/endoscope=average colony quantity of two dish×20

Pathogen monitoring: shacking the sample liquid at least 80 times, and inoculate 0.2ml to 90mm blood dish, Chinese blue dish(中国兰平皿) and SS dish, daubing it equally, then put it into 35C incubator for 48 hours, observe if there have pathogen growing.
